The RN Case Manager is responsible for coordinating and managing the delivery of quality, cost-effective care for patients across the healthcare continuum. The role involves clinical assessment, care planning, resource coordination, discharge planning, and patient advocacy to ensure positive health outcomes and efficient use of resources.

Key Responsibilities:

• Conduct comprehensive assessments of patients’ clinical, psychosocial, and functional needs.
• Develop individualized care plans with interdisciplinary teams.
• Coordinate care transitions, including discharge planning and referrals to home health, rehabilitation, or long-term care.
• Monitor patient progress, identify barriers to care, and implement interventions as needed.
• Communicate effectively with patients, families, providers, and external agencies.
• Ensure appropriate use of medical resources and advocate for the patient’s needs.
• Collaborate with physicians and healthcare team members to facilitate timely and safe patient discharges.
• Document all case management activities in the medical record in a timely and accurate manner.
