Your day to day as a Care ManagerAs a Care Manager at MonmouthCares, you will play a crucial role in meeting with families to assess their needs and coordinate care for children and youth facing mental health, behavioral health, and intellectual/developmental challenges. Your expertise and compassion will be instrumental in identifying the best resources and services to support these individuals, ensuring they receive the care and assistance they require to thrive. What matters mostTo excel as a Care Manager at MonmouthCares, candidates must possess a background as a social worker in social services or human services, demonstrating attention to detail and a strong understanding of deadlines. The role requires individuals to be reliable, responsible, and accountable in their duties, ensuring the highest level of care and support for children and youth in need. A comprehensive knowledge of the educational system, including special education, as well as familiarity with the child welfare, juvenile justice, and child protection services systems, is essential. A minimum of one year of experience working with at-risk children and their families is required to navigate the complexities of this important role effectively.
